Job Summary:

The Licensed Practical Nurse (LPN) will provide high-quality care to our home care clients, ensuring their physical and emotional well-being. The LPN will be responsible for administering medications, performing various nursing duties, and collaborating with other healthcare professionals to ensure seamless care delivery.

Responsibilities:

• Conduct focused assessments of clients and report results to the Registered Nurse (RN)
• Develop and implement individualized care plans in collaboration with the RN and other healthcare professionals
• Administer medications and treatments as prescribed by the RN
• Provide education and training to unlicensed staff members on medical procedures and policies
• Maintain accurate and up-to-date client records, including electronic charting
• Collaborate with the RN and other healthcare professionals to ensure continuity of care
• Participate in mandatory annual in-service training and attend required meetings

Requirements:

• Graduate of a school of nursing for Licensed Practical Nurses
• Current unencumbered license with the Minnesota Board of Nursing
• At least one year of experience in home care or long-term care, preferably
• Excellent judgment and decision-making abilities, with strong problem-solving skills
• Ability to work independently and as part of a team, with strong communication and interpersonal skills
• Knowledge of state requirements for Comprehensive Home Care license and LPN scope of practice
• Ability to use computers and electronic records, with basic computer knowledge
• Ability to effectively communicate in English, both verbally and in writing
• Must be able to read, comprehend, and follow instructions, with accuracy in all job activities

Working Conditions:

• Primary responsibilities will be conducted in clean, well-lit, climate-controlled areas, such as client apartments, hallways, work rooms, client dining areas, and meeting rooms
• Exposure to body fluids, infection, odors, and behavior of clients that may be aggressive, combative, or demanding
• Moderate noise levels, with possible exposure to chemical and respiratory hazards

Physical Demands:

• Work hours as assigned, scheduled, or mutually agreed upon, with occasional short-notice work
• Regularly required to use hands and fingers to examine clients, handle or feel objects, tools, or controls
• Frequently required to walk, bend, stoop, push, pull, kneel, reach overhead, and lift and/or move up to 25 pounds, with occasional lift and/or move up to 100 pounds
• Must have adequate vision and hearing necessary to perform job functions
