Job Summary

We are seeking an experienced and independent optometrist to lease space inside our Walmart stores. As an independent lease holder, you will have the autonomy to establish your own practice, including setting your days of operation, hours of operation, patient schedule, and fee schedule. You will also have the opportunity to opt into equipment and instrumentation, as well as administrative and/or opt-tech support services, for a fair market value.

Responsibilities
  • Provide eye and vision care services to patients, including comprehensive eye exams and vision correction services.
  • Establish and maintain a patient schedule, including setting appointment times and managing patient flow.
  • Set your own fee schedule and keep 100% of exam/professional fees.
  • Have the opportunity to opt into equipment and instrumentation, as well as administrative and/or opt-tech support services, for a fair market value.
Requirements
  • Doctor of Optometry (OD) from an accredited optometry school.
  • Valid state optometry license.
  • General Liability Certificate.
  • Professional Liability Insurance Certificate.
  • W-9.
Preferred Qualifications
  • Ophthalmologist (MD or DO) from an accredited school.
  • Valid state ophthalmology license.
