Job OverviewThis is an exciting opportunity to join our team and make a meaningful difference in the lives of our residents. As a Care Manager, you'll be responsible for building strong relationships with our residents and their families, providing assistance with daily living activities, and ensuring their individual needs are met.
Responsibilities- Provide personalized care and support to a designated group of residents, including assistance with daily living activities, medication management, and mobility support.
- Collaborate with our healthcare team to develop and implement Individualized Service Plans (ISPs) that cater to each resident's unique needs and preferences.
- Build strong relationships with residents and their families through regular communication, empathy, and compassion.
- Participate in ongoing training and education to enhance your skills and knowledge in eldercare and dementia care.
- Maintain accurate records and reports on resident care, including progress notes, incident reports, and other documentation as required.
To succeed in this role, you'll need:
- A high school diploma or equivalent.
- Previous experience working with seniors, preferably in a care or support role.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with residents, families, and colleagues.
- Ability to work independently and as part of a team, with a flexible approach to adapting to changing situations.
- Basic computer skills, including Microsoft Office and electronic medical records systems.
- Availability to work various shifts, including evenings, weekends, and holidays.

- An estimated salary range of $45,000-$65,000 per annum, based on location and experience.
