Advanced Practice Clinician

Key Responsibilities:
- Conduct comprehensive in-home assessments to identify patient needs and develop personalized care plans
- Perform evidence-based practice screenings and provide education to patients on disease management and wellness
- Collaborate with healthcare teams to ensure seamless care coordination and communication
- Identify and address health disparities and social determinants of health to improve patient outcomes
Requirements:
- Active NP or PA license or ability to obtain by start date
- For NPs - Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification OR for PAs - Physician Assistant national certification through NCCPA
- Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating various dwelling conditions
- Reliable transportation to complete home visit assessments
Preferred Qualifications:
- 1+ years of clinical experience in a healthcare setting
- Experience in gerontology, cardiology, internal medicine, or endocrinology
- Home Health care or home visit experience
