Job Summary

We are seeking a highly skilled and compassionate Brain Injury Case Manager to join our team. As a key member of our team, you will work closely with the BI Program Manager to house and serve individuals who have experienced a brain injury, have little to no income, and may otherwise experience homelessness.

Key Responsibilities
  • Assist the BI Program Manager in providing daily oversight of case management activities, including guidance on service delivery, documentation, and compliance with regulatory standards.
  • Provide BI services as needed to program participants, assisting individuals with daily living activities, interpersonal skills, and problem-solving.
  • Collaborate with other providers, care coordination, and community resources to ensure comprehensive support.
  • Maintain accurate and up-to-date documentation in electronic files, including intake forms, assessments, treatment plans, and progress notes.
  • Provide excellent customer service, responsiveness, and achieve high rates of housing stability and consumer outcomes.
Requirements
  • Bachelor's degree in human services or current student enrolled in a university or college working towards a degree in human services.
  • Brain Injury Specialist certification (CBIS) preferred.
  • Knowledge of neurological conditions, including traumatic or acquired brain injury recovery, and community resources.
  • Certification in CPR and First Aid or ability to become certified within six months of hire.
  • High level of organization skills, including attention to detail, planning, prioritizing, and meeting deadlines.
  • Excellent communication skills, including verbal, written, presentation, and listening.
