Key Responsibilities
  • As a Case Manager RN, you will be responsible for coordinating patient care and advocating for their needs while developing appropriate discharge plans.
  • You will review patient records and collaborate with families and healthcare teams to determine the best course of action.
  • Maintaining communication with patients and their families throughout their hospital stay is essential.
Qualifications

To be considered for this role, candidates must meet the following requirements:

  • Possession of a valid Texas RN License.
  • Current BLS certification.
  • A minimum of 2 years of clinical experience, ideally in inpatient Behavioral Health or home health case management.
  • An Associate Degree in Nursing (ADN) is required.
