Overview
This role involves delivering patient care, coordinating health services, and promoting wellness through education and coaching on disease prevention and chronic care management.
Key Responsibilities
- Collaborate in the formulation, execution, assessment, and modification of policies and guidelines.
- Exhibit leadership and performance that enhances care for outpatients in specialty settings.
- Conduct clinical and administrative tasks, utilizing nursing skills to assess and triage patients, whether in person, via phone, or through secure messaging.
- Ensure accurate and timely documentation in patient medical records and related documents.
- Employ the nursing process and evidence-based practices to work alongside healthcare providers and caregivers in developing comprehensive care plans.
- Participate in a collaborative team approach to identify patient concerns, categorize acuity levels, and facilitate appropriate interventions.
- Oversee and guide the work of Licensed Vocational Nurses (LVNs) and Certified Nursing Assistants (C.N.As), and may take on charge nurse responsibilities.
Specialty Clinics
The nurse will serve in various specialty clinics, which may include areas such as Infectious Disease, Allergy, Dermatology, and more, catering to diverse patient populations with complex health needs.

Qualifications
Applicants must possess a valid nursing license and meet the educational requirements set forth by the accrediting bodies. Experience in specialty clinics or triage is preferred.
