Hospice Nurse Care Manager

Job Summary:
LHC Group is seeking a skilled Registered Nurse (RN) to fill the role of Hospice Patient Care Manager. This position requires a minimum of two years of full-time experience working as an RN, with a focus on hospice care. The ideal candidate will have excellent leadership and communication skills, with the ability to motivate and inspire others to provide exceptional care.
Responsibilities:
- Lead, inspire, and motivate others to provide exceptional care.
- Ensure all staff are oriented and supported to be successful in their job performance.
- Liaise between staff, patients, and the medical community.
- Assist the Executive Administrator/Executive Director/Director of Clinical Operations with patient care activities.
- Comply with hospice policies and procedures, as well as local, state, and federal regulatory requirements.
- Actively promote and direct the team regarding quality of care and safety of patients, as well as staff.
- Engage in thorough problem resolution and complaint investigation.
- Provide in-services to staff to promote health and safety, familiarize them with regulatory issues and agency policy, and address identified educational needs.
Requirements:
- A minimum of two years of full-time experience working as an RN.
- Two years of full-time clinical experience in hospice care as a licensed practical nurse may be substituted for the required two years of experience as a registered nurse.
