Riverpark Operations LLC seeks a highly skilled Physical Therapist to join our team at Avamere Riverpark of Eugene. The successful candidate will provide top-notch patient care, while working collaboratively with our passionate and purposeful therapy team.
Salary: $83,000 - $110,000 per year, based on experience.
About the RoleWe are looking for a dedicated Physical Therapist who is passionate about delivering exceptional patient outcomes. As a member of our in-house therapy team, you will have the opportunity to work with patients and residents, their families, and fellow team members to create a culture of excellence and compassion.
Main Responsibilities:
- Develop and implement personalized physical therapy plans to meet the unique needs of each patient.
- Conduct thorough evaluations and assessments to identify areas for improvement and set achievable goals.
- Collaborate with other healthcare professionals to ensure seamless care coordination and optimal patient outcomes.
- Provide education and support to patients and their families to promote empowerment and self-management.
- Maintain accurate and comprehensive documentation of patient progress and treatment plans.
To be successful in this role, you must possess:
- A graduate degree in Physical Therapy from an accredited program.
- A valid state license to practice as a Physical Therapist.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations.
