Job Summary

We are seeking a highly skilled and compassionate Case Manager to join our team at HOPE Community Services, Inc. The successful candidate will be responsible for providing case management services to individuals in need of behavioral health and/or co-occurring services.

Key Responsibilities
  • Provide Clinical Services: The Case Manager will provide clinical services to address Substance Use Disorder/Co-Occurring needs to individuals involved in Addiction Recovery Services Programs.
  • Coordinate Client Needs: The Case Manager will coordinate the needs of clients and family members with resources available and provide client advocacy as needed.
  • Assist with Community Resources: The Case Manager will assist clients in finding and accessing community resources needed to maintain independent functioning.
  • Develop Recovery Plans: The Case Manager will assist clients in developing an individualized recovery plan that focuses on personal and interactive problems, independent living skill needs, defining goals, and to plan action reflecting interests, abilities, and needs.
  • Establish Rapport with Clients: The Case Manager will establish rapport with clients, assessing client needs; organize, plan, and deliver face-to-face client services; and conduct follow up on the outcome of services provided.
  • Document Services: The Case Manager will utilize the philosophy of Collaborative/Therapeutic Documentation to meet clinical record keeping documentation standards and ensure documentation is completed with the Consumer to the extent possible and all documentation is completed on the same day of the date of service.
  • Interact with Other Professionals: The Case Manager will interact with other professionals to discuss therapy or treatment, new resources or techniques, and to share information.
Requirements
  • Education and Experience: A Bachelor's degree in psychology, social work, or other related mental health field is required. The Case Manager must have experience in providing case management services to individuals with behavioral health and/or co-occurring disorders.
  • Certifications and Licenses: The Case Manager must obtain and maintain DMHSAS CM Certification. The Case Manager may be required to obtain and maintain First Aid and CPR Certifications.
  • Transportation: The Case Manager may be required to provide client transportation using personal vehicle or agency owned van or small bus.
Working Conditions

The Case Manager will work in a fast-paced environment and may be required to work evening or weekend hours. The Case Manager will be responsible for maintaining strictest confidentiality guidelines for interactions regarding consumer and family information.
