Job Summary

We are seeking a compassionate and skilled Home Health Aide to join our team at Summit Home Care in Queens. As a Home Health Aide, you will provide essential care and support to our patients in the comfort of their own homes.

Responsibilities
  • Provide daily personal care, including bathing, grooming, and oral care
  • Assist with daily living activities as outlined in the Plan of Care
  • Monitor patients and report on their condition
  • Help with patient meals and feeding, as well as basic housekeeping functions
  • Accompany patients to doctor's visits, shopping, and other essential errands
Requirements
  • New York State Home Health Aide Certificate
  • Government-issued ID, Social Security Card, and Work Authorization (if needed)
  • Two Recommendation Letters
  • Recent Physical
  • TB Screening (Quantiferon preferred)
  • Flu shot
  • Rubella & Rubeola titer lab work
  • Chain of Custody Drug Screen (no older than 6 months)
