Job Overview

Key Responsibilities

The key responsibilities of this role include:
- Coordinating patient care and collaborating with an interdisciplinary team
- Conducting quality assessments and developing plans of care for each patient
- Providing education and support to patient caregivers
- Maintaining accurate clinical records and participating in quality improvement initiatives
- Demonstrating flexibility and composure in times of crisis and adapting to changing patient needs

Requirements

To succeed in this role, you will need to possess a RN license in the state of requested employment (or eligibility to obtain), along with 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology. You should also be able to demonstrate a Hospice heart - being compassionate, kind, and empathetic to your patients and their families.
