Primary Care Physician

We are seeking a dedicated and compassionate healthcare professional to join our team at LifeLong Medical Care. As a Primary Care Physician, you will play a vital role in providing high-quality outpatient primary care to our patients.
Responsibilities- Provide preventive, urgent care, and primary care health services to patients
- Perform clinical assessments, order diagnostic evaluations, and plan programs of care
- Provide verbal and written education and counseling to patients, families, and caregivers
- Maintain patient records necessary for clinic reporting, billing, and quality improvement
- Participate in a team-based care model and attend clinical team meetings
- Coordinate care with other health team members, including case management and behavioral health staff
- Act as an advocate for patients and liaison with outside agencies as appropriate
- Current California MD, DO, NP, or PA license
- Training and experience in geriatrics, internal medicine, and/or family practice with board eligibility or certification
- Interest in working with a diverse patient population in a community-based organization and sensitivity to their needs and health problems
- Experience with electronic health records
- DEA and CAQH PAVE/ORP
- Community Health Center experience
- Experience as part of a team-based care model
- EMR Epic experience
