Job Title: Licensed Practical Nurse

We are seeking an experienced and compassionate Licensed Practical Nurse (LPN) to join our team at Crestview Operations LLC in Portland, OR. As a key member of our healthcare staff, you will provide high-quality patient care, work collaboratively with our nursing team, and contribute to the overall well-being of our residents.

About the Role
- Complete required record keeping in regards to admitting, transferring, and discharging residents
- Coordinate with nursing staff and support personnel daily to plan services, programs, and activities for residents
- Review resident care plans for appropriate resident goals, problems, approaches, and revisions based on nursing needs

Requirements
- Must possess a nursing degree from an accredited college or university
- Must possess a current, unencumbered, active license to practice as an LPN in this state
- Knowledgeable of nursing and medical practices, procedures, laws, regulations, and guidelines that pertain to nursing care facilities
