Job Description

Job Summary:

The Lifestyle Coach is responsible for implementing the Diabetes Prevention Program under the direction of the Nurse Case Manager. The coach will work with participants to provide education, training, and support to help them achieve their health goals.

Key Responsibilities:

  • Develop and implement the Diabetes Prevention Program curriculum
  • Provide education and training to participants on healthy lifestyle habits, including nutrition and physical activity
  • Work with participants to set and achieve health goals
  • Provide ongoing support and coaching to participants
  • Collaborate with the Nurse Case Manager to ensure program goals are met
  • Monitor and report on program progress and outcomes

Requirements:

  • Bachelor's degree in a related field
  • Experience working in a healthcare setting
  • Knowledge of diabetes prevention and management
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team

Work Environment:

This position requires the ability to work in a fast-paced environment and to prioritize multiple tasks and responsibilities. The Lifestyle Coach will work closely with the Nurse Case Manager and other healthcare professionals to ensure the success of the Diabetes Prevention Program.
