Job Summary

Harmony Home Health is seeking a skilled Occupational Therapist to join its team of healthcare professionals in Contra Costa and Alameda counties. As an Occupational Therapist, you will play a vital role in helping patients regain independence and improve their quality of life.

Responsibilities
  • Evaluate patients' functional status, including muscle function, endurance, visual coordination, and communication skills.
  • Develop treatment plans and establish goals for improved function, communicating with referring physicians and other healthcare professionals.
  • Teach patients new skills or retrain them in daily activities lost due to illness or injury, in accordance with organizational policy.
  • Fabricate splints and instruct patients on the use of adaptive equipment to improve function.
  • Maintain accurate clinical records, notes, and reports, submitting them in accordance with organizational policy.
  • Collaborate with other healthcare personnel to ensure coordinated and efficient care.
Requirements
  • Active Occupational Therapy License in California
  • Home Health experience preferred
  • Ability to prioritize tasks, manage time effectively, and maintain a high level of organization.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Dependable transportation, current driver's license, and auto insurance.
  • Current CPR certification and ability to perform the physical aspects of the position.
  • Complete TB test and physical examination.
