As a team member of our HouseCalls product, you will play a vital role in providing annual clinical assessments to patients in the comfort of their homes.

Key Responsibilities:
  • Conduct in-home assessments, including medical history review, medication reconciliation, vital signs, and comprehensive physical exams.
  • Perform evidence-based practice screenings, including point-of-care testing, as appropriate.
  • Identify diagnoses to inform care management and active medical management.
  • Communicate findings to inform members' primary care physicians of potential gaps in care.
  • Intervene in urgent and emergent situations, as needed.
  • Educate members on disease processes, medication, and compliance.
Requirements:
  • Active NP or PA license or ability to obtain by start date.
  • Licensure must be unencumbered, free of any open/unresolved disciplinary actions.
  • For NPs, active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care certification.
  • For PAs, Physician Assistant national certification through NCCPA.
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating various dwelling conditions.
  • Reliable transportation for home visit assessments.
Preferred Qualifications:
  • 1 year of clinical experience in the highest level of education, clinical setting.
  • Experience in Gerontology, Cardiology, Internal Medicine, or Endocrinology.
  • Home Health care or home visit experience.
  • Ability to travel up to 5% within licensed states, if requested for additional incentives.
