Job DescriptionJob Summary: We are seeking a skilled Registered Nurse to join our team at Summit Health Inc. As a Registered Nurse, you will be an integral part of a multidisciplinary care team, providing individualized, team-based care to patients. You will work closely with healthcare professionals to assess, plan, implement, and evaluate patient care, ensuring the highest level of quality and patient satisfaction.
Key Responsibilities:- Assist physicians and staff with daily patient care coordination and access.
- Function as a liaison between patients, staff, and physicians, ensuring seamless communication and care.
- Maintain efficient patient flow, prioritizing tasks and ensuring timely completion of patient care activities.
- Deliver direct patient care within the scope of licensure and in accordance with Summit Health Inc. policies.
- Collect lab specimens, accurately labeling and processing them for analysis.
- Perform Point of Care testing, EKGs, and phlebotomy as applicable.
- Prioritize patient care by implementing physician orders and nursing procedures.
- Ensure proper preparation for patient visits, preparing pertinent information and materials as needed.
- Manage nurse visit schedules, preparing necessary information and materials.
- Perform patient intake, check-in/check-out as needed.
- Effectively communicate problems, concerns, or issues to the Office Supervisor/Manager promptly and appropriately.
- Assure all patients are correctly identified prior to providing and/or documenting care.
- Employ timely and appropriate use of Tasking in EHR.
- Manage Clinical Nurse/Assigned inbox.
- Obtain vital signs (BP, temp, pulse, height & weight) accurately and completely, documenting in EHR.
- Administer injections according to protocol, documenting accurately in EHR and administering VIS sheets.
- Perform patient outreach to satisfy quality measures (BP checks, vaccinations).
- Coordinate and facilitate daily huddles.
- Appropriately demonstrate telephone triage protocols and document in EHR.
- Schedule appointments accurately and in accordance with department guidelines.
- Identify and schedule annual visits as applicable.
- Monitor daily hospital discharge list, collaborating with Care Management for TCM visits.
- Adhere to universal safety precautions when administering medications and disposing of medical waste.
- Review and update medication lists in EHR, demonstrating understanding of prescription control and refill procedures.
- Initiate and follow through with prior authorization requests, DME orders, and home health referrals.
- Enter orders and schedule appointments in the Practice Management System (PMS) and EHR.
- Provide referrals and/or consult orders appropriately and in a timely manner in the PMS and EHR.
- Check and maintain logs for emergency equipment according to policy.
- Facilitate transition of care to UCC/Hospital.
- Maintain proficiency with department-specific equipment, troubleshooting and reporting malfunctions promptly.
- Actively participate in pilot programs.
- Assist in ordering and maintaining of all supplies as assigned.
- Demonstrate flexibility with various work schedules.
- Perform other duties as required.
- Pushing and pulling, taking frequency and weight into consideration.
- Physical strength to lift heavy objects, carts, or items, taking frequency and weight into account.
- Physical mobility, which includes movement from place to place on the job, taking distance and speed into account.
- Physical agility, which includes ability to maneuver body while in place.
- Dexterity of hands and fingers.
- Balance is maintained during climbing, bending, and/or reaching.
- Endurance (e.g., continuous typing, prolonged standing/bending, walking).
