As a Palliative Care Nurse Practitioner, you will play a vital role in providing high-quality patient care and support to individuals in need of palliative care services.
Responsibilities- Provide skilled nursing assessment, planning, and provision of patient care to maximize the palliation of symptoms and determine eligibility for care.
- Identify patient or family needs and provide supportive care and appropriate services in accordance with the attending physician's orders, the interdisciplinary team plan of care, and the policies and procedures of the agency.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients and their families.
- Conduct regular assessments and evaluations to ensure that patients are receiving the highest level of care and to identify any changes in their condition.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination and to address any concerns or questions.
- Licensed as a Nurse Practitioner/Advanced Practice Nurse.
- Experience in hospice and/or palliative care is preferred but not required.
- Ability to work full-time hours and be available for on-call shifts as needed.
- Current and valid CPR certification, Driver's License, and auto insurance.
