Healthcare Navigator

The Primary Health Network is seeking a skilled Digital Health Navigator to join our team. As a key member of our healthcare team, you will play a vital role in ensuring that individuals with challenges using technology related to healthcare have access to the necessary skills to utilize.

This position will require you to meet clients at their individual need and skill level to provide personalized and small group training sessions. You will also be responsible for building individual and community capacity by increasing knowledge and self-sufficiency through activities such as outreach, community education, and social support.

Key Responsibilities:

• Provide personalized training sessions to clients with varying skill levels
• Develop and implement outreach and community education programs to increase knowledge and self-sufficiency
• Collaborate with healthcare teams to ensure seamless delivery of services
• Stay up-to-date with the latest technology and healthcare trends to provide effective support

Requirements:

• Associate's degree or equivalent combination of education and experience
• Strong computer skills and experience with healthcare technology
• Presentation and facilitation skills
• Exceptional communication and customer service skills
• Ability to work independently and as part of a team
