Job OpportunityWe are seeking a Nurse Practitioner or Physician Assistant to join our team for PRN/Part Time Home Risk Assessments. As a member of our team, you will conduct adult/geriatric assessments, medical history, diagnosis, and treatment, as well as provide health education and referrals.
Responsibilities- Conduct in-home assessments and medical history reviews
- Diagnose and treat patients as needed
- Provide health education and referrals to patients and families
- Collaborate with primary care physicians and other healthcare professionals
- Work in a variety of settings, including home visits, skilled facilities, and telehealth
- Master's degree in nursing or physician assistant studies
- Current RN and NP or PA licensure in the state of practice
- Board certification in a Matrix-approved specialty
- 1 year of experience as a Nurse Practitioner or Physician Assistant
- Strong computer skills and familiarity with EMR software
- Excellent verbal and written communication skills
- $95.00 per completed assessment
- Mileage reimbursement plan
- Flexible scheduling
