About the Role

We are seeking a skilled Licensed Practical Nurse to join our team at DaVita, a leading provider of dialysis care. As a key member of our healthcare team, you will play a vital role in delivering high-quality patient care and making a meaningful difference in the lives of our patients and their families.

Key Responsibilities
  • Conduct thorough patient assessments, reviewing vital signs and lab results to determine the best course of treatment.
  • Develop and implement individualized care plans, working closely with patients and their families to ensure their needs are met.
  • Administer medications and treatments as prescribed by physicians, monitoring patient responses and adjusting care plans as needed.
  • Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes.
Requirements
  • Current LPN license in the state of practice.
  • At least 12 months of clinical nursing experience, with 3 months of experience in dialysis care.
  • Basic computer skills and proficiency in MS Word and Outlook.
