Rehabilitation Services Therapist

At SolutionHealth, we are committed to delivering high-quality patient care and rehabilitation services. As a Physical Therapist in our Inpatient Department, you will play a vital role in helping patients achieve optimal outcomes.
The role of the Physical Therapist within our organization is focused on the delivery of rehabilitative services. You will evaluate, plan, and implement specific treatment programs for patients according to the principles and practices of the specific discipline and setting (acute care or outpatient).
Your key responsibilities will include:
- Evaluating the skilled therapy needs of referred patients and developing a written prioritized plan of care, including short- and long-term rehabilitation goals developed in collaboration with the patient and/or other members of the healthcare team.
- Preparing for patient evaluation by conducting medical record review, identifying areas of focus, contraindications, and precautions. Ensuring orders for therapy are active and appropriate.
- Updating treatment plans, re-evaluating patients, and communicating with referrers and other clinicians involved in the patient's care.
- Establishing and implementing discharge plans and instructing patients, family members, and other essential personnel in effective performance of home exercise programming or other strategies to maximize function.
Requirements
To be successful in this role, you will need to have a graduate degree from an accredited Physical Therapy Program and corresponding entry-level degree. You must also be licensed as a Physical Therapist by the state of New Hampshire Physical Therapy Governing Board and possess CPR certification. Previous experience in a clinical setting is preferred but not required.
We offer a competitive salary range of $60,000 - $80,000 per year and a comprehensive benefits package that includes health, dental, prescription, and vision coverage for full-time and part-time employees.
