Job Opportunity

We are seeking a skilled and compassionate Healthcare Specialist to join our team at Meridian Health Services.

About the Role:

As a Healthcare Specialist, you will work closely with our clinical team to provide skills and development training to patients and families, helping them achieve developmental milestones and reduce psychosocial barriers.

Key Responsibilities:

  • Develop, implement, and revise individual treatment plans in collaboration with the Therapist.
  • Provide individual and group skills training and development to assist patients and families in developing skills such as self-care, daily life management, interpersonal communication, and problem-solving.
  • Work collaboratively with the Therapist and medical staff to ensure coordination of integrated care for patients, including identification of health issues, compliance with treatment, and effective patient-to-medical-provider communication.
  • Complete CANS/ANSA re-assessments and provide consultation to staff on specific cases involving coordination of care.
  • Maintain positive working relationships with staff of other agencies and institutions, and act as a liaison between patients and agencies and/or institutions.

Requirements:

  • High school diploma or equivalent with 2 years of paid behavioral healthcare experience or equivalent college coursework toward a human services or health-related degree.
  • Associate or Bachelor's degree preferred.
  • A valid driver's license with a driving record meeting Meridian's driving policy, reliable transportation, and proof of auto insurance.
  • A commitment to providing individual attention to help clients achieve mental, emotional, and physical well-being.
