Job Title: LPN Murfeesboro GallatinJob Summary

Evergreen Life Services is seeking a skilled Licensed Practical Nurse (LPN) to join our team in Murfeesboro, TN. As an LPN, you will play a vital role in providing high-quality care to individuals with intellectual and developmental disabilities.

Responsibilities
  • Contribute to the planning and implementation of daily projects to ensure the health and well-being of individuals served.
  • Update the Medical Program Coordinator and other staff regarding health-related problems and discuss treatment plans.
  • Assist in educating staff on individual health conditions and participate in staff education.
  • Assess individual health needs and record findings on nurse notes.
  • Participate in medical QA checks and contribute to the Individual Support Plan (ISP).
  • Attend ISP and Circle of Support meetings as requested.
  • Monitor and report seizures to the RN Healthcare Coordinator.
  • Serve as the Liaison nurse for individuals served, assisting in making nursing decisions and judgments.
  • Demonstrate knowledge of infection control procedures and communicate with physicians as needed.
  • Accompany individuals to physician visits and obtain necessary information.
  • Administer medications according to best practice guidelines and follow MAR and Physician orders.
  • Provide tube feeding treatments and other skilled nursing treatments as needed.
  • Review and maintain health-related documentation and generate incident reports as needed.
Requirements
  • Graduate of an approved Licensed Practical Nursing Program.
  • Minimum of one year clinically relevant experience, preferably working with elderly or intellectually disabled individuals.
  • Knowledge of medical nursing procedures, first aid, and basic principles of nursing.
  • Ability to learn about intellectual and developmental disabilities, seizure disorders, Autism, and other associated diagnoses.
