Clinical Excellence & Patient Care
Deliver exceptional patient care by utilizing your knowledge and clinical skills to provide care for individuals with physical disabilities and chronic illnesses.
Participate in patient and family educational activities, and implement individualized teaching and discharge plans to promote patient independence and goal achievement.
Perform hands-on nursing care, utilizing the nursing process to achieve quality outcomes for patients.
Reinforce care provided by other team members, and provide direction and supervision to unlicensed assistive personnel as needed.
Coordinate nursing care activities with other members of the interdisciplinary rehabilitation team to facilitate achievement of overall goals.
Develop effective communication skills to ensure the fulfillment of requirements for legal documentation and reimbursement.
Maintain a strong commitment to the multidisciplinary healthcare team, and contribute to the assessment of patients' physical, psychological, sociocultural, and spiritual dimensions.
Collaborate with other rehabilitation team members to develop and implement the interdisciplinary plan of care.
Implement the plan of care within the scope of LPN practice, and provide nursing care and education directly or through ancillary personnel as needed.
Contribute to the evaluation of nursing care and modify the plan as needed to achieve measurable goals and objectives.
Participate in interdisciplinary team conferences and meetings, and offer input into team decision-making within your scope of practice.
Intervene with team members and other healthcare professionals to ensure optimal opportunities for patient recovery.
Collaborate with staff nurses and other team members to achieve cost-effective care by utilizing appropriate clinical measures to meet emergent physical, psychosocial, and spiritual situations.
Communicate effectively and interact in a manner that facilitates group process and team building.
Advocate and support patients' rights to make informed care decisions.
Practice reflects knowledge of federal and state regulatory and accrediting agencies.
Clinical Excellence & Patient Education
Reinforce information provided to patients about disease processes underlying disabilities, and teach nursing techniques to help patients and families develop self-care skills necessary to move toward wellness.
Provide assistance to patients and families within your scope of practice for future self-management and decision-making responsibilities.
Reinforce teaching done by staff nurses and other specialists, and provide resource materials for patients' changing needs.
Demonstrate knowledge that patient and workplace safety is a priority by preventing errors, accidents, and infections, and reporting potential or actual concerns immediately.
Qualifications
Education
Successful completion of a Licensed Practical Nurse program from an accredited college.
Experience
Experience in Post-Acute Care or an Acute Care environment.
License(s)/Certification(s)
Current licensure in good standing to practice as a Licensed Practical Nurse within the state you are practicing.
Must possess and maintain current Basic Life Support (BLS) certification or obtain certification within three (3) months from date of hire.
