Job Description:

We are seeking an experienced Registered Nurse to join our team as a Nurse Care Coordinator in Fort Worth, TX. As a key member of our nursing staff, you will be responsible for ensuring the provision of quality care that promotes the highest practicable physical, mental, and psychological well-being of each resident.

Key Responsibilities:

  • Observe, assess, document, and report resident condition/changes during rounds
  • Assure implementation of care plans as directed by physicians and other healthcare professionals
  • Administer medications as prescribed by physicians
  • Conduct regular rounds with physicians and record visits in clinical records
  • Perform narcotic reconciliation with incoming and outgoing shifts
  • Identify significant changes in the condition of residents and take necessary action per facility policies and procedures
  • Oversee and monitor certified nursing assistants (CNAs)
  • Initiate and complete Incident and Accident Reports per facility policy
  • Discharge residents per facility policy
  • Communicate with resident's point of contact regarding status updates or changes in condition

Qualifications:

  • Educational Requirements: Completion of a RN program at an accredited vocational school, college, or university
  • Licensing Requirements: Active Registered Nurse license in the state of Texas in good standing
  • Experience Requirements: Experience working in a skilled nursing facility preferred

Benefits:

We offer a competitive salary range of $60,000 - $80,000 per year, based on experience. Additionally, we provide a comprehensive benefits package, including health insurance, retirement plan, and paid time off.
