About the Role

As a Healthcare Advocate LPN at Sunrise Senior Living, you will serve as a key member of our healthcare team, advocating for the needs and interests of our residents. With a strong focus on patient-centered care, you will be responsible for developing and implementing individualized care plans, collaborating with healthcare professionals, and communicating effectively with residents and families.

We offer a competitive salary of $75,000 - $90,000 per year, depending on location and experience, plus benefits that include medical, dental, and vision insurance, 401(k) matching, and paid time off.

Responsibilities
  • Assess the health needs and preferences of each resident, developing and implementing individualized care plans that meet their unique requirements.
  • Collaborate with healthcare professionals, including physicians and therapists, to ensure comprehensive and coordinated care delivery.
  • Communicate effectively with residents, families, and healthcare team members to ensure seamless care delivery and resolve any concerns promptly.
  • Participate in regular training and development opportunities to enhance your skills and stay up-to-date on best practices in senior care.
Requirements
  • A current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
  • Minimum of one (1) year experience in home health, assisted living, hospital, or long-term care environment.
  • Demonstrates excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare team members.
  • Possesses a strong understanding of nursing practices, techniques, and methods applied to health and wellness resident care.
