As a RN Case Manager with the Good Samaritan Society, you will play a vital role in coordinating and integrating healthcare services for clients from pre-admission to post-discharge. This involves sequencing and coordinating care to enhance patient/resident flow and promote interdisciplinary communication, leading to early intervention and improved outcomes.
Key Responsibilities- Integrate healthcare services for clients across various settings, including structured and unstructured healthcare environments.
- Utilize independent nursing judgment to coordinate and sequence care, ensuring seamless transitions and optimal patient/resident outcomes.
- Collaborate with healthcare teams to evaluate and trend patient/resident responses, utilizing structured care methodologies and protocols.
- Monitor and control variances and duplication, ensuring fiscal responsibility and efficient resource allocation.
- Communicate effectively with patients/residents, family members, and healthcare teams to coordinate care and promote patient-centered care.
- Bachelor's degree in nursing preferred.
- Graduate from a nationally accredited nursing program, including Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).
- Minimum of one to two years of professional nursing experience in a clinic care setting, with in-depth knowledge of specialty area.
- Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state.
