Wraparound Facilitator

Overview:

Responsibilities:

• Communicate effectively with customers, colleagues, and other stakeholders to answer questions, provide information, and address concerns.
• Coordinate team meetings and facilitate the wraparound process to ensure progress and document all relevant information.
• Develop and implement ongoing working plans for the team, meeting with members to ensure progress and providing education on the wraparound process.
• Ensure all team members receive copies of the plan of care, including the youth involved, and maintain team cohesiveness.
• Educate members on wellness, recovery, and resiliency, and connect families with needed community resources.
• Work closely with local MAP teams for flexible funding and meet with clients and team members regularly.
• Attend and participate in bi-weekly treatment team meetings and serve as a liaison with other professionals and agencies.
• Actively recruit new referrals to maintain a full caseload and provide 24/7 on-call support for clients as required.

Qualifications:

• Bachelor's degree in a human services discipline, including social work, psychology, sociology, or counseling, and experience working with children and families in community support services.
• Must attend 3 training courses via the WRAPAROUND institute to become a certified Wraparound Facilitator and begin credentialing process through DMH as PCSS.
