The successful candidate will provide direct support to individuals in both home and community settings, ensuring their personal goals and outcomes are achieved in accordance with our policies and procedures.
- Key Responsibilities:
- Provide one-on-one support to individuals, assisting them in achieving personal goals and outcomes.
- Implement each individual's Community Life Plan, ensuring services and activities align with their personal preferences and goals.
- Accompany individuals on recreational, social, and community outings, providing guidance and support as needed.
- Facilitate and engage individuals in leisure activities such as arts, crafts, games, and other meaningful interactions in the home setting.
- Provide general assistance to individuals in their home and/or community setting to implement activities of daily living.
- Document client interactions, progress, and activities in accordance with company policies.
- Maintain open lines of communication with supervisory staff, addressing individual concerns and reporting progress.
- High School Diploma or G.E.D.
- At least 18 years of age.
- Valid driver's license and insured personal vehicle.
- Ability to meet transportation requirements as outlined in the job duties.
We offer a competitive salary of $45,000 - $55,000 per annum, depending on experience.
