About the Role

The successful candidate will provide direct support to individuals in both home and community settings, ensuring their personal goals and outcomes are achieved in accordance with our policies and procedures.

  • Key Responsibilities:
  • Provide one-on-one support to individuals, assisting them in achieving personal goals and outcomes.
  • Implement each individual's Community Life Plan, ensuring services and activities align with their personal preferences and goals.
  • Accompany individuals on recreational, social, and community outings, providing guidance and support as needed.
  • Facilitate and engage individuals in leisure activities such as arts, crafts, games, and other meaningful interactions in the home setting.
  • Provide general assistance to individuals in their home and/or community setting to implement activities of daily living.
  • Document client interactions, progress, and activities in accordance with company policies.
  • Maintain open lines of communication with supervisory staff, addressing individual concerns and reporting progress.
Qualifications and Requirements
  • High School Diploma or G.E.D.
  • At least 18 years of age.
  • Valid driver's license and insured personal vehicle.
  • Ability to meet transportation requirements as outlined in the job duties.

We offer a competitive salary of $45,000 - $55,000 per annum, depending on experience.
