Job Overview

This opportunity involves working as a member of our multidisciplinary team to provide rehabilitative care and skilled nursing services to adult and geriatric clients in their homes. You will be responsible for conducting thorough assessments, developing individualized care plans, and implementing evidence-based interventions to promote client independence and overall well-being.

Responsibilities
  • Client Assessment: Conduct thorough assessments of clients to identify their needs and develop effective care plans.
  • Care Planning: Develop and implement individualized care plans tailored to each client's unique needs and goals.
  • Interventions: Implement evidence-based interventions, such as manual therapeutic exercises, gait training, and balance therapy, to promote client independence and mobility.
  • Documentation: Accurately document all assessments, interventions, and client progress in our state-of-the-art electronic medical records system.
Requirements
  • A current NJ Physical Therapist license.
  • A minimum of one year of recent work experience as a Physical Therapist.
  • Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills; prior experience with electronic medical records (EMR) preferred.
