LPN - Long Term Care - Days

Job SummaryThe LPN provides patient-centered care to our residents, working collaboratively with the interprofessional team to deliver high-quality care and achieve excellent clinical outcomes. This role requires a strong commitment to compassion, excellence, leadership, innovation, and agility, and a passion for delivering exceptional patient care.
Key Responsibilities- Collect data on the physical condition and nursing needs of patients, and assist with implementing the plan of care.
- Collaborate with the interprofessional team to provide the highest quality of care and clinical outcomes.
- Perform delegated tasks using evidence-based practice and specific competencies.
- Identify ways to improve the patient's experience of care, streamline care processes, and lower costs while promoting quality.
- Contribute to a professional environment that encourages mentoring, engagement, and development to retain expert clinicians.
- Maintain positive relationships with physicians and other interprofessional team members, collaborating to problem solve and to improve patient care.
- Demonstrate, anticipate, and proactively manage risk to prevent crises.
- Current Wisconsin LPN licensure.
- American Heart Association Healthcare Provider Basic Life Support (BLS) certification.
- Frequent exposure to sharp objects and instruments.
- Occasional exposure to moving mechanical parts, fumes or airborne particles, toxic or caustic chemicals, and risk of electrical shock.
- Occasional high noise level in work environment.
- Standing and/or walking for extended periods of time.
- Transporting patients and/or equipment from one location to another; little likelihood for injury if proper body mechanics and procedures are followed.
- Possible exposure to communicable diseases, hazardous materials, and pharmacological agents.
- Occasional contact with aggressive and/or combative patients.
