Nurse Liaison

Ascension is seeking a skilled Registered Nurse to serve as an Intake Liaison for our Home Health Care team. In this role, you will be responsible for coordinating home care and hospice intake activities, explaining services to patients and families, and facilitating continuity of care.

Key Responsibilities:

• Coordinate home care and hospice intake activities and resources
• Complete referral process
• Explain home health and/or hospice services and facilitate continuity of care
• Assist with communication or payer sources and determine benefit eligibility
• Obtain, record, and disseminate referral information
• Document clinical records to ensure compliance with regulatory standards, agency policies, and procedures

Requirements:

• Registered Nurse credentialed from the Illinois Board of Nursing
• Diploma from an accredited school/college of nursing or required professional licensure at time of hire
• Prior case management experience preferred
