Job SummaryThe Physical Therapist will be responsible for providing physical therapy services to patients in their homes, adhering to the conditions outlined in the Service Employment Agreement. This role requires a strong clinical background, excellent communication skills, and the ability to work independently.
Key Responsibilities- Assess and evaluate patients' therapeutic, rehabilitative, and functional status, and participate in the development of the total plan of care.
- Provide physical therapy services to patients, including assessing muscle strength, mobility, gait, ROM, and transfer capabilities.
- Direct physical therapy treatment, instruct patients and families/caregivers in the use and care of therapeutic appliances, and determine priority needs for physical therapy.
- Report to physicians on patients' reactions to treatment or changes in condition, and initiate physical therapy programs.
- Participate in patient discharge planning, prepare and submit clinical and progress summaries, and provide physical therapy consultation to families/caregivers as needed.
- Doctoral Degree in Physical Therapy (DPT) from a program approved by an accredited organization in Physical Therapy.
- Licensed to practice as a physical therapist within the state.
- At least two years of appropriate experience as a physical therapist, with Home Health and/or Hospice experience preferred.
- Demonstrates good verbal and written communication, and organization skills.
- Possesses and maintains current CPR certification.
