Key Responsibilities:

• Conduct initial and ongoing clinical evaluations to assess patient needs and functional levels, including OASIS assessments at designated intervals.

• Collaborate with physicians and care-team members to develop and update individualized Plans of Care.

• Administer therapeutic interventions and assess equipment requirements as necessary.

• Evaluate care outcomes and facilitate appropriate discharge planning.

• Implement infection control measures and standard precautions during patient care activities.

• Accept clinical assignments aligned with your qualifications and expertise.

• Supervise Physical Therapy Assistants (PTA) and Home Health Aides (HHA) as required.

• Complete necessary documentation promptly.

• Fulfill continuing education requirements mandated by the agency and licensing board, and engage in in-service training.

• Utilize effective communication skills to consult with families and other agency personnel as needed.

• Uphold the agency's philosophy and administrative policies.

• Demonstrate commitment to professional development and competency.

• Proficiently use technology to facilitate communication and perform essential job functions.


Qualifications:
Education:
• Bachelor of Science in Physical Therapy or higher from an accredited program recognized by the American Physical Therapy Association.

Licensure:
• Current Texas State License as a Physical Therapist and a valid Texas Driver's License.

Skills:
• Proficient therapy skills in accordance with established standards of practice. Strong interpersonal abilities. Current BLS certification for healthcare providers. Familiarity with durable medical equipment.

Transportation:
• Reliable transportation and valid auto liability insurance.

Experience:
• Preferred experience in Home Health and manual therapy; knowledge of HomeCare HomeBase is a plus.
