Hospice Registered Nurses play a vital role in providing compassionate care to patients and their families. As a Hospice RN Case Manager, you will be part of an interdisciplinary team that delivers high-quality care and support.

Key Responsibilities:

  • Provide in-home care to patients with six months or less to live, focusing on quality of life
  • Work collaboratively with a physician, LPN, CNA, HHA, social worker, chaplain, and volunteers to carry out the patient's plan of care
  • Perform patient assessments, check vitals, evaluate breathing, monitor pain, administer prescribed medication and treatments
  • Ensure accessibility of medical equipment, prescribed oxygen, medication, and other supplies
  • Educate family members on the patient's prognosis, changes in care plan, medication, and hospice goals
  • Take a holistic approach to care that engages the patient's mind, body, spirit, and family

Requirements:

  • Graduate of an accredited nursing program and active RN license in Virginia
  • Minimum of 2 years nursing experience, ideally in hospice or home healthcare
  • CPR certification
  • Knowledge of state and federal home health regulations
  • Clinical proficiency in nursing specialty, strong communication skills, and compassion
  • Valid Driver's license and/or transportation

This position requires a mandatory on-call rotation.
