">Responsibilities:
">As a Hospice Care Specialist, your primary responsibilities will include:
">- ">
- Evaluating Patient Needs: Assessing patients' medical conditions, health history, and developing personalized plans of care to ensure their comfort and well-being.">
- Collaborating with Interdisciplinary Teams: Working closely with nurses, doctors, social workers, and other healthcare professionals to coordinate patient care and address complex needs.">
- Patient Education and Support: Educating patients and their families on preventative care, medical management, and emotional support to help them navigate the end-of-life journey.">
- Documentation and Record-Keeping: Maintaining accurate and detailed records of patient interactions, assessments, and interventions to ensure seamless care coordination.">
- Communication and Teamwork: Communicating effectively with patients, families, and healthcare teams to ensure clear understanding of patient needs and preferences.">
Requirements:
">To be successful as a Hospice Care Specialist, you must possess:
">- ">
- Nursing Licensure: Current licensure as a Registered Nurse (RN) in the state.">
- Advanced Practice Nursing: A Master's degree in nursing with a minimum of one year of experience as an Advanced Practice Nurse (APN).">
- Hospice Experience: A minimum of one year of experience in hospice or palliative care.">
- Certifications: Accreditation from an approved certifying body for advanced practice nursing and specialized certifications in areas such as hospice and palliative nursing (CHPN), pediatrics, or geriatrics.">
- Soft Skills: Excellent observation, verbal, and written communication skills; problem-solving skills; mathematical skills; and nursing skills per competency checklist.">
- Physical Demands: Prolonged or considerable walking or standing; able to lift, position, and/or transfer patients; able to lift supplies and equipment; and considerate reaching, stooping, bending, kneeling, and/or crouching.">
- Transportation: Must be a licensed driver with an automobile that is insured in accordance with state/organization requirements and is in good working order.">
Estimated Salary: $120,000 - $150,000 annually, depending on location and experience.
