Job Summary

We are seeking experienced Home Health Registered Nurses to join our team in the South Philadelphia territory. This is an excellent opportunity for a skilled RN to become a valued partner in patient care, working at the top of their license and making a positive impact in their community.

Key Responsibilities
  • Provide high-quality patient care through the application of the nursing process, prioritizing patient safety and well-being.
  • Collaborate with the interprofessional healthcare team to coordinate care across the continuum, ensuring seamless transitions and optimal patient outcomes.
  • Participate in quality improvement initiatives, utilizing data and evidence-based practices to drive continuous improvement and excellence in patient care.
  • Develop and maintain strong relationships with patients, families, and caregivers, providing education and support to promote informed decision-making and empowerment.
  • Stay up-to-date with the latest clinical guidelines, research, and best practices, applying this knowledge to improve patient care and outcomes.
Requirements
  • Diploma or Associate Degree in Nursing (ASN) from an accredited school of nursing (Clinical Nurse-NCL track) or Bachelor of Science in Nursing (BSN) from an accredited school of nursing (Clinical Nurse 2 track).
  • 15+ months of relevant professional nursing experience, with a strong focus on home health care.
  • Pennsylvania (or New Jersey) RN licensure, with a valid driver's license and car insurance.
  • BLS/CPR certification as a healthcare provider, as per the American Heart Association.
