Educational Background and Experience

Qualifications:

Must hold an LPN or RN degree.

Experience

At least two years of experience in customer service is required.

One year of experience in healthcare or home care is preferred.

A minimum of one year of scheduling experience is necessary.

Key Responsibilities

  • Input and manage data for new and existing patient and employee records in the database.
  • Coordinate patient scheduling and track completed supervisory visits in accordance with regulatory standards.
  • Inform the clinical supervisor and case manager when verbal orders are required for start-of-care visits outside the standard timeframe.
  • Ensure satisfaction for both patients and clinicians by maintaining consistency in scheduling.
  • Handle requests for service and scheduling modifications while managing patient and resource calendars.
  • Report any staffing needs and performance concerns related to agency staff availability to the Clinical Supervisor as necessary.
  • Notify the appropriate departments regarding additional payer authorization for ongoing services prior to scheduled visits.
  • Communicate with the Care Coordinator at the initiation of service and document missed visits in the Electronic Medical Record as per protocol.
  • Record interactions with patients, families, or caregivers in the Electronic Medical Record through clinical notes.
  • Provide support to other team members as assigned.
  • Engage in information management and performance improvement initiatives.
  • Participate in annual training programs and mandatory educational seminars.
  • Stay informed about new methods, products, and trends, sharing relevant information with staff.
  • Perform additional administrative tasks as assigned.
  • Enter scheduling data and create and maintain staff schedules, ensuring client appointments align with care plans and staff availability.
  • Communicate with care providers and clients regarding daily scheduling changes.
  • Verify that caregivers possess the necessary licenses and certifications.
  • Review documentation to ensure all forms are completed accurately.
  • Ensure that files are comprehensive with all required documents.
  • Review client information and care plans upon receiving new clients, placing them in an unassigned schedule for monitoring and staffing progress.
  • Assign appropriately skilled personnel for new clients and promptly input scheduling information into the database after caregiver confirmation.

Compensation: $16 - $18 per hour
