Job Summary:

The Nurse Care Specialist will be responsible for providing skilled nursing care to patients in their homes, under the supervision of a Registered Nurse. This includes assessing patient needs, developing and implementing care plans, and providing education and support to patients and their families.

Key Responsibilities:
  • Provide skilled nursing care to patients in their homes, under the supervision of a Registered Nurse
  • Assess patient needs and develop and implement personalized care plans
  • Provide education and support to patients and their families
  • Collaborate with other healthcare professionals to ensure comprehensive patient care
  • Document patient care and progress in a timely and accurate manner
Requirements:
  • Graduate of an accredited school of nursing
  • Current, unrestricted state license as a Licensed Nurse in the state of practice
  • Current CPR certification
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures
Preferred Qualifications:
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting
  • Home health experience
Physical Requirements:
  • Must be able to speak, write, read, and understand English
  • Must be able to travel
  • Must be able to lift 50 pounds
  • Must be able to sufficiently reposition patients and move equipment without assistance
Work Environment:
  • Must be able to function in a wide variety of environments, which may involve exposure to allergens and other various conditions
  • Possible exposure to blood, bodily fluids, and infectious diseases
