Job Summary

The Symbii Home Health and Hospice is seeking a highly skilled Clinical Speech Language Pathologist to join our team. As a key member of our home health care services, you will be responsible for implementing standards of care for speech language pathology services and ensuring adherence to all conditions in the Service Agreement.

Duties & Responsibilities
  • Conduct thorough evaluations, assess the patient's home environment (as necessary), and identify equipment needs relative to speech/language function.
  • Assume responsibility for ongoing interdisciplinary assessment and development of individualized plans of care in partnership with patients, representatives (if any), and caregivers.
  • Provide services as ordered by physicians, as indicated in the plan of care.
  • Patient, caregiver, and family counseling.
  • Patient and caregiver education.
  • Preparing clinical notes.
  • Communication with physicians and other healthcare practitioners related to the current home health plan of care.
  • Identifying functional speech defects and establishing plans to improve patient function.
  • Administering speech therapy programs utilizing specialized techniques and/or equipment.
  • Recommending communication devices/aids as necessary.
  • Participating in case conferences, in-services, and other meetings required to ensure coordinated and comprehensive care.
  • Maintaining clinical competency in speech language pathology practice and theory.
  • Identifying patient and family/caregiver needs for other services and referring accordingly.
  • Supervising home health aides in procedures directly related to the speech-language therapy plan of care.
