Job Description:

The MDS Nurse Coordinator plays a crucial role in ensuring the successful implementation of our care programs and services. As a key member of our interdisciplinary team, you will be responsible for coordinating the development and completion of resident assessments, ensuring compliance with regulatory requirements, and collaborating with healthcare professionals to achieve optimal outcomes.

Responsibilities:

  • Coordinate the development and completion of resident assessments, including MDS, CAA's, and comprehensive care plans.
  • Ensure compliance with regulatory requirements, including federal, state, and local laws and regulations.
  • Collaborate with healthcare professionals to achieve optimal outcomes and improve resident care.
  • Communicate effectively with residents, family members, and healthcare professionals to ensure seamless coordination of care.
  • Develop and maintain effective relationships with healthcare professionals, vendors, and other stakeholders.

Requirements:

  • Educational Background: RN Nursing Degree or LVN from an accredited school or college.
  • Licensure: Valid RN or LVN license in good standing.
  • Certifications: Valid CPR and BLS card.
  • Qualifications/Experience: Minimum 1 year of skilled nursing experience preferred.
  • Language Skills: Must be able to speak, read, write, and comprehend the English language.
  • Technical Skills: Ability to use a computer/tablet to enter resident data.
  • PCC Proficiency: A plus.

Salary: The estimated salary for this position is $65,000 - $85,000 per year, depending on experience.
