**Geriatric Medicine Opportunity**
We are seeking a skilled Geriatric Medicine Physician to join our team in Augusta, GA. As a Geriatric Medicine Physician, you will provide high-quality care to patients with acute and chronic illnesses encountered in the older adult population.
Responsibilities- Provide comprehensive care for patients with complex health needs, including assessment, diagnosis, treatment, management, education, and health promotion.
- Lead a care team consisting of care promoters, care facilitators, and care coordinators to ensure seamless patient care.
- Engage with hospitalists and transitional care teams to ensure continuity of care for patients in the hospital, SNF, LTC, or homebound.
- Take an active role in the management of your center and contribute to the recruitment of patients and additional providers as needed.
**Requirements**
- Board certification in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty.
- Current, active State Medical License for the state(s) in which you will be working.
- Basic Life Support (BLS) certification.
- Current DEA number for schedule II-V controlled substances.
