Job Summary

Fresenius Medical Center is seeking a highly skilled and dedicated Patient Care Specialist to join our team. As a Patient Care Specialist, you will play a vital role in providing safe and effective dialysis therapy to patients under the direct supervision of a licensed nurse.

Key Responsibilities
  • Patient Care
    • Assist patients with their dialysis treatment, ensuring their safety and comfort throughout the process.
    • Evaluate patients' vascular access and perform cannulation as needed.
    • Monitor patients' vital signs and report any changes or concerns to the registered nurse or supervisor.
    • Administer heparin as delegated or as allowed by state law.
  • Treatment Preparation
    • Prepare patients for dialysis treatment by taking their vital signs and evaluating their vascular access.
    • Initiate dialysis treatment according to prescribed orders, including blood flow and dialysate flow.
    • Monitor patients' response to dialysis therapy and report any changes or concerns to the registered nurse or supervisor.
  • Documentation and Communication
    • Accurately document patients' treatment data, including vital signs and treatment parameters.
    • Communicate effectively with patients, families, and healthcare team members to ensure seamless care.
  • Continuous Quality Improvement
    • Participate in Continuous Quality Improvement (CQI) activities to enhance patient care and outcomes.
    • Collaborate with the Clinical Manager and Education Coordinator to drive organization quality standards.
Requirements
  • Education and Certification
    • High school diploma or GED required.
    • Valid PCT national certification or state-specific certification as defined by Center for Medicaid/Medicare Services (CMS).
  • Experience
    • At least two years of previous patient care experience in a hospital setting or related facility preferred.
  • Skills
    • Excellent communication and interpersonal skills.
    • Ability to work effectively in a fast-paced environment.
    • Strong attention to detail and ability to maintain accurate records.
