Job Summary

We are seeking a skilled Home Health Registered Nurse to join our team at Choice Health at Home. As a Home Health RN, you will provide high-quality nursing care to patients in their place of residence, coordinating care with the interdisciplinary team, patient, family, and referring agency.

Key Responsibilities
  • Conduct comprehensive assessments and develop individualized plans of care to ensure quality and continuity of care.
  • Implement and document the plan of care, recommending revisions as necessary.
  • Consult with and educate patients and families on self-care techniques, nutrition, and dietary needs.
  • Provide clinical directions to Home Health Aides and LPNs/LVNs to ensure quality and continuity of service.
  • Prepare clinical and progress notes that demonstrate progress toward established goals.
  • Communicate effectively with patients, families, team members, and other healthcare professionals.
Requirements
  • Current RN License
  • Valid Driver's License
  • CPR Certification
  • Valid Auto Insurance
  • At least 2 years of Home Health RN experience
  • HomeCare HomeBase experience preferred
