Job Description

The Registered Dietitian Certified Diabetes Educator will play a critical role in providing nutrition and diabetes management services to improve health outcomes, quality indicators, and patient experience for our patients. This position will collaborate with primary care physicians, endocrinologists, RN Care Managers, and other interdisciplinary team members to identify at-risk patients, provide evidence-based nutrition and disease-specific educational interventions, coordinate care with other team members, and prevent avoidable utilization and adverse outcomes.

Essential Job Functions:
  1. Demonstrate broad knowledge of nutritional approaches to common disease states such as hypertension, diabetes, hyperlipidemia, obesity, and heart failure.
  2. Responsible for managing the assessment and learning needs of patients with nutritional issues and/or diabetes, recognizing and integrating patients' special needs and individualizing care as appropriate.
  3. Assess the patient's and/or family's ability to understand and comply with nutritional education and instruction and modifies interventions accordingly.
  4. Provide comprehensive nutritional care to outpatients in a professional, courteous, effective, and prompt manner, promptly addressing patient phone calls and questions.
  5. Collaborate appropriately with physicians, RN Care Managers, and other members of the interdisciplinary team, referring appropriately to meet patient needs.
  6. Accommodate urgent problem visits and consults as needed, handling emergent teaching needs such as new insulin starts, meter teaching, insulin pump malfunctions/review.
  7. Display competency in nutrition and diabetes education and all aspects of documentation, coding, and billing relevant to the scope of service.
  8. Proficient with the use of EHR, including visit documentation and signing of notes in a timely manner, employing timely use of EMR and patient portal to enhance communication with patients and colleagues.
  9. Document patient assessment/reassessment and care plans, conducting interventions according to policy and evidence-based guidelines.
  10. Provides diabetes counseling and education to patients and families, including blood glucose monitoring, medication administration, nutrition plans, and foot, eye, and dental care.
  11. Collaborates with Population Health/Care Management team for timely patient follow-up with PCP and/or specialist and entry into complex case management as appropriate.
  12. Identifies those patients with Social Service needs, making appropriate social work referrals.
  13. Recognizes behavioral health needs, facilitating timely referrals.
  14. Identify, update, and utilize educational resources to achieve optimum healthcare outcomes.
  15. Communicate and document change in patient's condition to all appropriate individuals.
  16. Document patients' medication use, adherence, and concerns, monitoring effects of new medication regimens, recognizing complex medication regimens, poly-pharmacy, and high-risk medications, and facilitating timely referral to Ambulatory Clinical Pharmacist.
  17. Educate/train/triage patients on insulin pumps and advantages/challenges of use, as needed.
  18. Respond effectively in an emergency situation, addressing the patient's immediate needs.
  19. Provide follow-up services to patients who have received diabetes education through the program, reviewing glucose logs, answering questions about glucose meter, CGM, or insulin pumps, coordinating care with other team members.
  20. Communicate and provide care and treatment plan consistent with age, cultural, spiritual, and developmental needs of the patient.
  21. Assure appropriate and timely data collection for tracking and analysis of outcomes for process improvement purposes.
  22. As requested, facilitate and lead support groups to assist patients with behavior as well as personal lifestyle modifications.
General Job Functions:
  1. Other job duties as required.
  2. Maintains knowledge of current advances in the field of diabetes and completes at least 75 CE credits every 5 years.
  3. Fulfill annual CME requirements.
  4. Maintain NJ State License.
  5. Maintain professional memberships.
  6. Uses current technology to perform job duties, remaining up to date on all new diabetes-related equipment such as glucometers and insulin pumps.
  7. Works collaboratively and effectively with all SMG departments and services.
  8. Participates in community as nutrition and diabetes advocate and educator.
  9. Serves as a resource and consultant for physicians and nursing staff for special nutrition and diabetes needs.
  10. Promotes team atmosphere among colleagues and staff, working effectively with staff.
  11. Performs as a preceptor when and if necessary for new hires into the department.
  12. Acts as a patient advocate and upholds patient's rights, referring ethical concerns to appropriate resources.
  13. Must be able to travel between patient care locations.
Education, Certification, Computer, and Training Requirements:
  1. Registered Dietitian (Registration number and card).
  2. Diabetes Education Certificate required CDE certificate and card.
  3. Master's degree preferred.
  4. Minimum 2-4 years of related experience, one year of patient education experience preferred.
  5. Must have experience with managing insulin pumps and continuous glucose monitoring systems.
  6. Ability to communicate in English, both orally and in writing required.
  7. Strong interpersonal and organization skills required.
  8. Strong ability to perform diverse work assignments and make decisions with time limitations with a high degree of accuracy required.
  9. Strong problem-solving and critical thinking skills required.
  10. Strong multi-tasking and priority setting skills required.
  11. Experience with Standard Office Equipment (Phone, Fax, Copy Machine, Scanner, and Email/Voice Mail) required.
  12. Experience with Standard Office Technology in a Window-based environment preferred.
