We are seeking a highly skilled and experienced Advanced Care APP to join our team at DispatchHealth. As a key member of our innovative care delivery program, you will provide hospital-level care to patients in the comfort of their own homes.
Key Responsibilities- Deliver advanced level care to patients in their homes, including performing clinical assessments, developing and ordering hospital-level and post-acute level internal medicine clinical workups, and documenting patient care.
- Collaborate with our multidisciplinary team to create and implement comprehensive plans of care for patients on our Advanced Care, Extended Care, and Bridge Care service lines.
- Provide overnight call coverage as assigned and work closely with our Advanced Care clinical team to ensure seamless care delivery.
- Foster strong relationships with our DispatchHealth Advanced Care and Acute Care APPs, providing education and engagement around our Advanced Care program services.
- Participate in program enhancement initiatives for our Advanced Care, Extended Care, and Bridge Care service lines.
- Contribute to our commitment to quality, safety, and experience by continuously enhancing our Advanced Care services.
- Graduate with an advanced degree from an accredited institution.
- 2-3 years of hospitalist or hospitalist equivalent experience.
- Current unrestricted state license as a Nurse Practitioner or Physician Assistant.
- Nationally board certified, current.
- Current BLS / ACLS certification.
- Prescriptive authority and DEA.
- Ability to lift and carry equipment up to 50 pounds.
- Ability to walk up and down several flights of stairs easily while carrying equipment.
- Valid driver's license with clean driving record.
- Ability to work a varied schedule with evenings, holidays, and weekends required.
