Job Overview
We are seeking an experienced Occupational Therapist to join our team as a Home and Community Care Specialist. As a key member of our team, you will be responsible for providing functional rehabilitation services to clients in their natural environment, promoting greater health and independence through the recovery process.
Key Responsibilities:
- Perform evaluations and develop treatment plans tailored to each client's needs and goals.
- Regularly re-assess effectiveness of treatment plans and collaborate with the treatment team to ensure seamless care coordination.
- Communicate patient's needs and progress to the treatment team, physician, and family members.
- Maintain accurate records of client interactions, progress, and treatment outcomes.
Requirements:
- Minimum of a bachelor's degree in Occupational Therapy from an accredited program.
- Current, unrestricted license as an Occupational Therapist by state in which practicing.
- A minimum of one year's work experience as an Occupational Therapist.
- Demonstrates knowledge of rehabilitation techniques related to complex neurological diagnoses preferred.
- Ability to lift 50 pounds and perform duties that require fine motor skills, visual acuity, and walking/standing for extended periods.
