Behavioral Health Case Manager

As a Behavioral Health Case Manager at Kaiser Permanente, you will play a vital role in providing integrated and coordinated care to high-risk and high-utilizing populations in inpatient and outpatient psychiatry and addiction medicine settings. Your expertise will be instrumental in managing all facets of care and utilization of these patients, ensuring they receive the best possible outcomes.
Key Responsibilities:
- Initial Intervention: Engage with high-risk, high-utilizing patient populations, acting as a central point of contact for referrals from multiple departments.
- Assessment and Evaluation: Conduct thorough assessments and evaluations of clinical status, environmental resources, mental and intellectual capabilities, and networking skills.
- Service Planning: Develop personalized service plans based on assessments of short- and long-term needs.
- Tracking and Quality Management: Monitor referrals and outcomes to ensure quality management and continuous improvement.
- Consultation and Coordination: Operate as a consultant for other departments, facilitating collaboration between Primary Care, Behavioral Health Care Services, and Healthcare Providers.
- Linkage with External Resources: Establish linkages and maintain working relationships with outside resources to provide seamless care.
- Direct Patient Intervention: Provide individual and/or group therapy for chronic, high-utilizing patient populations, as well as individual supportive psychotherapy and family counseling when necessary.
- Crisis Assessment and Intervention: Respond to crisis situations, providing timely and effective interventions.
- Relevant experience in case management and working with community resources.
- At least two years of experience in mental health settings, including inpatient and outpatient treatment services.
- Masters degree in a Social Work, Psychology, Sociology, or other field related to behavioral health.
- Licensed Professional Clinical Counselor (California), Licensed Marriage and Family Therapist (California), or Licensed Clinical Social Worker (California).
